This session will build your confidence to discuss future care plans without undermining hope\, using structured communication techniques. You’ll learn how to recognise when to start the conversation\, navigate sensitive discussions with patients and families\, and align care with patient wishes—improving end-of-life care while reducing avoidable hospital admissions.. Participants will watch a real-time interactive demonstration led by EC4H tutors\, exploring effective ways to support conversations about future care planning with a patient who is marginalised. During the webinar will explore effective ways to help people think and plan ahead.
