Advance Care Planning: Making Your Voice Heard – North Middlesex University Hospital

We will set up a stall in the public area at North Middlesex University hospital to raise awareness about the importance of Advance Care Planning (ACP) discussions.
In addition, we will visit the adult wards in the hospital to encourage our colleagues to initiate ACP conversations with patients and their families, promoting early and meaningful care planning discussions.
