Bolton NHS Foundation Trust & Farnworth Care Home- What advance care planning

Bolton NHS Foundation Trust are jointly hosting an ACP event with Farnworth Care Home (part of Abbey Healthcare).  They have arranged a collaborative event for residents and families and system partners to come together, to talk about what advance care planning is, and to hear each others viewpoints.  They aim to understand any barriers from each of the system partners and to solutions together a way to remove these barriers to ensure that all provision is patient and family centric and meets their needs.

The event is spanning across the locality and is going to attended by our palliative care team and geriatricians, through to North West Ambulance Service and our out of hours providers. They will be ensuring the voice of the residents and their families are central to the whole event.  They are intending to use the outcomes to be not only raising the profile of advance care planning, and the benefits for people and their families, but also that we need to ensure as health professionals that we are facilitating the appropriate sharing of wishes across system partners to avoid unwanted conveyance to hospital but with providing the right services to support out of hospital including care closer to home.

It will be hosted at Farnworth Care Home, Church Street, Farnworth, Bolton, BL4 8AG on Wednesday 7^th May from 1pm until 3pm.
